I had an Outlook 2003 machine that insisted on running the Mailbox Cleanup at the start-up of an Outlook session.

This was connected to a server running Exchange 2003 however the none of the retention settings were set so there was no reason for Outlook getting this information from the Exchange end.

Next up I temporaily gave the user an empty mailbox and the mailbox cleanup kicked in again.

The final solution that fixed it was to delete and re-create the users Outlook Profile from the Control Panel Mail applet.

I did read other posts about changing registry keys etc but decided to try a new profile first and it worked.
